Insurance Benefits:
All primary members should have received a letter and yellow card from ATV Maine for Accidental Death & Dismemberment Insurance. If you have already seen an insurance representative, you DO NOT need to send in the yellow card. However, if you have not seen a representative yet, PLEASE FILL OUT and send in the card. If you have any questions regarding the insurance, please call Dan Mitchell at 562-8873.
Starting July 1st, 2004, we will be able to pick up additional insurance on our spouse or children (under age 18 living in the same household), for an additional $2.00 per member. If you would like to sign up for the additional information, please make sure to fill in all the information on your membership renewal form. We also need to know how many people are in your family,( living in the same household) whether or not you sign them up for the insurance. If you join as a Single member, we still need to know how many family members are living in your household. This information is being used for statistics by Pineau's so we can show how many people ATV Maine represent when we go to Augusta.
